I’m sometimes amused by women who tell me that they have “sensitive” skin—like it’s a badge of honor, proof-positive of their femininity. The classic symptoms:
- diffuse redness, irritation, especially in the T-zone
- tingling, tightening, dryness and flakiness
- reaction or discomfort to procedures or products—shaving, waxing, cosmetics
- blotchiness, swelling, edema, which sometimes can advance to rosacea

In fact, it’s a sign of weakness, and it’s sometimes caused by the use—or overuse of the wrong products, particularly moisturizers. It seems antithetical—a moisturizer was probably the first product that you used on your skin—and the one you trusted and used most frequently— but it could also be the one that harms and ages your skin the most—either accelerating skin aging, or inducing skin sensitivity.
With young and healthy skin, the cells in the dermis collect water from the food we eat and water we drink, and deliver that water to the surface of the skin. It’s a natural part of cellular nourishment and repair. That’s why it’s so important that we keep skin cells active and functioning normally. But when you apply a moisturizer to hydrate and plump the skin, the skin cells in the epidermis send a message to the cells in the dermis: slow down, we’re fat and happy up here. That causes the cells in the dermis to become lazy, go dormant, and the skin becomes drier, thinner and less elastic. At that point, the skin is compromised, and begins to lose its ability to function as a natural and protective barrier. That induces sensitivity and leads to redness, irritation, wrinkling and accelerated aging.
Crazy as it seems, moisturizers—which supposedly are calming and soothing—are frequently just the opposite. If you’re going to use a moisturizer, use one that suppresses irritation, that calms the skin, that contains some retinol to awaken dormant cells. And don’t ever use a moisturizer to the exclusion of products that activate and stimulate your skin.
Kick the moisturizer habit. Make sure that your skin is rejuvenating and repairing itself. It’s one of the best strategies for well-nourished, healthy skin.

hi.. im using complex a to peel my face together with the blender cream etc.. but i stop using complex a coz im very expose to sun and the peeling was too aggressive although i saw the result and it looks wonderful. is there any product that will peel my face that is not very aggressive?
Hi, Angeli. Complex A, to anyone unfamiliar with that name, is a form of retinoic acid (also sometimes known as Retin-A or Renova). I sell it in my own practice, and it causes the skin to peel, enabling newer, healthier skin cells to emerge and surface. So my questions for Angeli:
* how long did you stick with the Complex A. Although it varies with each individual, peeling only lasts about 3-12 weeks. It will get better, believe me.
* did you try reducing the frequency of application. Some people can’t tolerate application every night. Use it every other night.
* are you using sunscreen–SPF 15 or higher? It’s always important, but it’s especially critical when you’re using retinoic acid, because it makes your skin more sensitive to UV rays. Use a little Complex A, and a lot of sunscreen!

i used the complex a for about a month. the dermatologist said it should be last for 6 weeks but i stopped it. the result was ok i like to be honest except that sometimes it hurts like burning. but i know from the very first that i will encounter that. but i stop it. i still want to continue using it but i want the other kind of peeling if possible. and yes i am using the zo sunscreen protection. my last question would be, is everything that i had encounter is normal? coz if yes, i will continue using it coz i still i like the result. but i u will suggest me to see a dermatologist, id be willing too. i know you will find me very impatient coz i want to see the result quickly.

Thanks for your nice words about ZO Skin Health products. First, there are lots of symptoms of sensitive skin–blotchiness, dryness, irritation, redness, burning, sensitivity to cold. Generally, there are some common denominators–the epidermis (the outermost layer of the skin) is very thin and taut; and there’s an imbalance of water, lipids (oils) and protein. I like to call this condition “weak” skin. In order to strengthen the skin, we want to activate and stimulate the skin cells so that they are not so easily affected by the environment, stress, etc. Literally and figuratively, we want to strengthen the skin.
So, in answer to your question–I can’t say that the use of ZO products generally will cure your sensitivity–it’s important to use certain specific products, like the Ossential products, to stimulate the skin cells. The Ommerse products also use ingredients that will stimulate the skin cells (DNA repair enzymes, vitamins C and E, retinol, etc.) however that is not the primary purpose of them. With time, yes, your skin will be strengthened.
As for TE pads…. 2% salicylic acid can be irritating to some people. However, I added botanical extracts to this formula to help calm and soothe the skin. I asked some of my favorite fair-haired, red-headed patients who have sensitive skin, and was told that their reaction to the TE pads was very transient, and not problematic.

I’m glad that you’re happy with ZO!!
Just as there are many definitions of sensitive skin (redness, irritation) there are many causes. It can range from mild to severe. Sensitive skin can be caused by harsh or heavily fragranced products. So if you can eliminate some of the obvious causes, and it can be determined that the sensitivity is not caused by extrinsic factors, I can tell you that I have had tremendous success by activating and stimulating the skin, to reduce or eliminate sensitivity. Depending on what products you are using (are you using a retinoid, or a product with a high concentration of retinol?), and how frequently you are applying them (some people can’t tolerate applying the products every day, especially in the beginning), it may take six weeks or more. You may want to consider NuDerm (see a dermatologist or plastic surgeon)–at least for a few months.
I get very few people who complain about the Te pads. There are some botanical extracts in them to calm and soothe the skin, and reduce any reaction to the salicylic acid. If you apply them only once a day, you shouldn’t have any problem.

I have using all the products the ZO Skin health except the Exfoliating cleanser.
I use all the products of your hardest program every night and morning except Tpads (that i uses 2 times per week)and the Exfoliating polish (that i uses one or two times per week). My skin is very bright and more smooth.
I can not use Nuderm because i live in Spain, and i don’t know if any doctor use this products.
The last question, must i spend some time between the one product and the following product when i apply them?

Hi, Angela: It’s a good idea to make sure that a product is absorbed into the skin before applying a sequential product. The reason: the sooner active ingredients are absorbed, the sooner they will start functioning. You don’t want to allow products to sit on the surface of your skin. Luckily some of the new skincare products which are being developed contain nano-sized particles of active ingredients, so they can penetrate into the skin quicker and deeper–so they work all the better.

I have been using the ZO cleanser, exfoliating polish and sunscreen and love them. I have a question about moisturizers. Would it be all right to substitute essential oils or facial oils for a moisturizer at least during the day. Do oils cause the same problem that moisturizers do? Thank you
Sorry, Jan, I’m not going to give you one bit of encouragement about essential oils. I know people who use grapeseed oil, jojoba oil, avocado oil, you name it!!! But they have the same effect as moisturizers–or they’re worse!! They satiate the outermost layers of your skin, and then your own skin cells fail to capture moisture and deliver it to the surface of your skin. Be sure to use a product that stimulates your skin, and then it’s OK to use a moisturizer. But you can’t (or shouldn’t, anyhow) have one without the other.
No essential oils… please!

Thank you for telling me about the essential oils. I have another question. Is it OK to use Retin A over an AHA product. If somone were to use an AHA toner that contains glycolic and salicyclic acids, would it be OK to use Retin A over that? Would doing that deactivate the Retin A or would it help push the actives into the skin? Thank you for having this blog and taking time to answer questions.
Jan: Those are all good questions! Yes, it’s OK to use Retin A after applying an alpha hydroxy acid product, and doing so may well enhance the penetration, and thereby increase the efficacy. There’s a possible downside though–it may be irritating. You may want to test your skin first–avoiding areas that may be particularly sensitive–like the area around the eyes, the mouth, and the neck. Let me know how it works!

The skin around the eyes is very delicate, thin, and easily irritated. As for cleansing–Are you wearing eye make up, and removing that with a soft sponge or cotton balls before bedtime? Most facial products should not be applied too close to the eyes or the eyelids. That’s because the active ingredients in them can sometimes migrate. Are you using an eye cream that gently moisturizes and increases epidermal thickness? First, as for your bank account: I assume that you’re only half serious, but don’t swap the stress of financial concerns for the distress of aging! You have bought a lot of products, so let’s figure the best way to use them sensibly and correctly.
I also appreciate your asking for a recommended regimen. I have long been been opposed to people buying products (mine or anybody else’s!) and not using them properly. Not only are you potentially wasting time, money, but you could be duplicating some ingredients, while missing others.
Given your skin type and concerns, I would recommend:
AM (all daily, unless indicated otherwise)
Exfoliating Cleanser (use in the shower, if you wish)
Exfoliating Polish (use 2-3X/week, in the shower)
TE pads (these are not currently in your regimen but will help with T-zone oiliness and blackheads/whiteheads.)
Daily Power Defense (use 2-3X, week, alternating with the Ommerse Daily Renewal Crème)
Ommerse Daily Renewal Crème (use 4-5 days/week, alternating with the Daily Power Defense)
Intense Eye Repair
Oclipse Suncreen + Primer SPF 30
Since you’re inclined to black/whiteheads, you don’t want to use too many products, as they could clog your pores. It’s for that reason (and because I want you to make the best possible use of the products, and be sensitive to your budget, that I recommend alternating the Daily Power Defense and the Ommerse Daily Renewal Crème).
PM (all nightly)

Do you think i have bought too much and items that i dont really need since you are suggesting that i return either the GFS or RNR – which should i keep and which should i return? Or, would it help to use both (assuming we forget my remark on bank balance, money can be found again!).
As for my pigmentation problem, it is mostly on my cheek and nose areas. I have not yet tried any aggressive treatment/method to combat this.

Neither the Growth Factor Serum nor the Radical Night Repair is designed to correct hyperpigmentation. No over-the-counter product (that I know of!) can effectively correct discoloration–in order to do that, you need some prescription-strength ingredients, some time (it can be 6 weeks or more) and dedication (skin renewal–peeling, etc. is a part of the process). If you want to treat the pigmentation problems, I suggest that you see a dermatologist. If you live in the US, you can identify a physician trained in NuDerm from obagi.com. If you live in Asia, there are many professionals in Malaysia, Indonesia, Phlippines, Singapore, you can help you (and if you wish, I can get you some referrals).
Are you ready to address the hyperpigmentation problem now? Or do you want to wait 30 days? At this point, I would suggest that you continue with the regimen that includes both products, and consider different possible courses of treatment for the hyperpigmentation. I’ll be interested to know what a physician recommends for you.

Hi, Pukli: I’m responding to this on the blog because I think that your problem is the same as that experienced by many people. Did you go “cold turkey” when you switched from the NuDerm to the ZO Skin Health regimen? Did your physician recommend a maintenance regimen, gradually reducing the frequency of application and number of products, or did you just quit cold turkey? NuDerm suppresses pigmentation, activates skin cells, revs up the production of collagen and elastin. After the completion of the treatment regimen, you can’t just go completely “off the wagon”. Sure, you can and should introduce ZO Skin Health products into the regimen, but not to the total exclusion of NuDerm.
Can you send an email to info@zoskinhealth.com, tell us what products you are currently using, describe your skin condition, and we’ll recommend a regimen. It’s difficult to try to recommend products via cyberspace–I’m not attempting to second-guess the opinions of your physician. But we can at least point you in the right direction.

right now i am only using zo skin health products.I start with the exfoliating cleanser followed by ossential daily power defense ,renewal creme and the oclipse sunscreen and eye cream.i the pm i use the cleanse follwed by growth serum and overnite creme followed by radical nite repair and eye cream.i see more wrinkles now and more pigmentation and also dry skin.i am very disappointed i thought this would be better than nuderm.and also i use the exfoliating polish 3 times a week.i’m follwing the exact directions sent to me .

Pukli–Thanks for your input. Of course I arguably have more experience with NuDerm and ZO Skin Health than any other physician, however I’m not an expert on you, Pukli. So please understand that I base my opinions on my experience and working knowledge as both a formulator and physician, not as your personal physician. Are you seeing a medical professional? So since I am reticent to address your concerns directly, I will speak in generic terms.
First, some NuDerm products suppress the melanocytes that produce pigmentation (brown spots, melasma, freckles). Obagi NuDerm Clear, Blender and Tretinoin Cream are usually used in combination over an 18 week period and work together to restore a clear, even-toned complexion. The retinoic acid works to awaken the cells in the dermis, stimulate the production of collagen and elastin, and repair sun damage. NuDerm products are generally recommended daily for 18 weeks, then we decrease the frequency of application, and begin introducing ZO Skin Health.
As you have observed, ceasing the NuDerm products completely may cause some spots and uneven skin tone to return.
I suggest that you see a physician, and ask get professional advice about resuming NuDerm for a limited duration. When you’ve achieved the results that you want, and are ready to maintain those results, I’ll be happy to advise you on how to combine NuDerm and ZO Skin Health product lines for a lifetime of healthy, younger looking skin.
I’m sorry, Pukli, and I don’t know if this will give you comfort, but there are many people like you who completed their regimen,and then didn’t go on to a NuDerm maintenance regimen. I hope you’re successful, and hope to hear that your skin has returned to optimal health.

As for the redness and peeling. I like to think of skin as a canvas. To change the art, you have to start with a clean canvas, and with skin, that’s sometimes requires redness and peeling. Rest assured, that’s a good thing, and will be temporary. Stick with the four products in the kit for now, then let’s revisit your skin care concerns after you have been treated for melasma.

I was so glad to find this blog and to know that you have an OTC line. I’ve been using NuDerm for almost 16 weeks now, including the 10% Vit C serum. My melasma, which has given me so much grief for so many years, has improved a lot and the texture of my skin has become smooth, I love it! However, the redness and flaking continues and sometimes the dryness, too. I thought that by this time my skin will be used to the active ingredients but it seems like I’ve developed full blown rosacea (without the pustules). Before starting on NuDerm I had sensitivity and reactivity problems already perhaps from trying different moisturizers throughout the years. My dermatologist tells me that the rosacea-like symptoms I’m experiencing is will always be there as long as I’m using the “bleaching products”. She suggested using the blender/tretinoin .025% twice a week only but when I did (last week), I noticed some darkening of the patches beginning to reappear. I must point out that I’m very vigilant about using the 35 SPF sunscreen, staying out of the sun and wearing a wide-brimmed hat if I have to be out during the day.
Is there any way I can incorporate the Rosaclear into the NuDerm? Like using the moisturizer perhaps after the Vit C serum in the morning instead of using the Clear and continuing the nightly regimen?
I feel like my dermatologist isn’t giving me other alternatives and need your help in leading me to an alternative protocol/Obagi products. Please let it not be a choice between melasma and rosacea…that will truly be heartbreaking!!

My question is if you can send me or recommend a product that can help repair previous damage (sun, bad moisturizers, etc). Is there a peel I can do? In your blog I read about complex A and blender cream. Can I order that online? And also: do you have a mask available, my skin is still a little dry.

That’s great that you’re experiencing great results with ZO Skin Health.
But if you have skin that makes you look like you’re in your mid-40’s you may want to try some of the products that you have mentioned. Complex A is the name that I use for a form of retinoic acid, but Complex A is sold only in my practice in Beverly HIlls (and some of my other practices throughout the world). Other similar products are called tretinoin and Retin-A.
Blender is the name for a particular NuDerm product that is used in combination with tretinoin to enhance penetration and repair environmental damage to the deeper layers of the skin. It will reduce (or eliminate entirely) the pigmentation that you describe. It has other benefits as well!
Yes, these products can be bought online, but I encourage you to see a physician (for one near you, check out obagi.com). It’s important for a physician to give you proper instructions and supervision. These are all medical grade products with physician-strength ingredients. You don’t want to take any chances; please see a professional. AFter you complete the treatment, let me know and I can help you incorporate the ZO Skin Health products into your maintenance regimen.
As for masks–no, there isn’t a mask in the ZO regimen, nor is there one in the NuDerm regimen. There are products in the ZO line, however, that will make your skin feel much more soft, hydrated, and provide barrier protection (they also contain retinol to stimulate the skin cells). Are you using Ommerse Overnight Recovery Creme?

Allow me to clarify. ZO Skin Health products don’t have an acne system that is separate and distinct. It’s primarily an anti-aging line.
Therefore, someone who has serious, chronic or cystic acne and inflammation should be using some other medical procedure or prescription medication.
But ZO Skin Health definitely has products that are recommended, and are effective for oily/acne-prone skin. Women who have hormonal acne, or just oily skin can be treated very successfully. I would particularly recommend:
ZO Skin Health Exfoliating Cleanser
ZO Skin Health Exfoliating Polish (2-3 x/week)
ZO Skin Health Te Pads (1-2X/day)–reduces sebum production, deep cleanses pores
As for the DNA kit, only one of these products (the cleanser) is contained in the DNA kit. So it wouldn’t be my first choice for you, if you’re concerned about oily skin or occasional breakouts. The DNA can, however, be used in combination with the other two products that I have recommended above. It’s good to use both the chemical (Te pads) and physical (Polish) exfoliants together for the treatment of mild acne.
Unfortunately, I don’t know the nature or extent of your acne, but mild to moderate acne can very successfully be treated with over-the-counter, non-medical products.

Hi I am interested in using the ZO products and have been given some samples to try.
I have dry skin which I normally smother in cream!
My friend uses the Obago Nuderm products but think they may be too strong for me.
I am 42 with a couple of crows feet and a bit of crepyness under my eyes. My skin is genaerally good but I have noticed if i look really hard that it is no longer the same colour all over and I just want to get back (if possible) a more youthfull look.
Now please sort that!! I have used exfoliating polish followed by night repair then growth then recovery and the eye renewal.
what regime would you suggest? I do work outside most days.
Hi, Nicola. I was concerned when I saw that you said that “smother” your face in creams. It’s not good to use products to excess. Let your skin absorb the product before applying a sequential layer. In a PS that you sent separately, I see that you have no whiteheads or blackheads, so that’s good–you haven’t clogged your pores–at least not yet.
The slight discoloration that you’re seeing doesn’t seem to be a serious concern yet. If the pigmentation irregularities become an increasing problem, consider the Obagi NuDerm products. As for your friend’s remark that NuDerm may be too strong. It’s used for virtually all skin types, we just need to vary the amount and frequency based on your tolerance.
In the meantime, keep using products that stimulate your skin, and use sunscreen religiously. In your PS, it looks like you buy your ZO products from Harrods. Even in England, where the sun is not intense, UV protection is critical. Here’s the regimen that I would recommend. I want you to use the products that you have effectively, and regularly. Therefore I’m not recommending a lot of other products.
Morning (7X/week unless otherwise indicated)
Cleanser formulated for Dry Skin. A hydrating cleanser is important for your skin type.
Exfoliating Polish. 2-3 X/week, in the shower.
Ossential Daily Power Defense. This will work well in combination with the sunscreen to not only prevent future damage but correct previous damage.
Eye Cream
Oclipse SPF 30 sunscreen (or use another SPF 30 sunscreen, but be sure to use it every day).
Evening
Cleanser formulated for Dry Skin.
Ossential Growth Factor Serum
Radical Night Repair
Ommerse Overnight Recovery Creme (if your skin feels like it needs extra moisturization)
Eye Cream

NUDerm and ZO products work under the same activate/stimulate philosophy, but the NuDerm products are of Rx strength, and require a period of irritation, dehydration, renewal. So what you are experiencing should not be the same as what your friend has seen/felt.
The ZO products that I have recommended should not leave you desperate for moisture. In the mornings, apply the Daily Power Defense–it has anti-oxidants that work to repair skin damage, as well as moisturize the skin. Stick with the Ommerse Overnight Recovery Creme in the evenings. With this regimen you should not be experiencing any discomfort. If you continue to have problems, please let me know.

I don’t know any of these doctors personally, but these have been recommended to me. Talk to them regarding their recommendations on how they would downregulate the melanin production. Hydroquinone (which is in NuDerm) is the best active ingredient for the treatment of pigmentation problems, but other procedures (lasers or NuDerm) may be required.

I’m still awaiting a reply to my message of 8 April but had to send this follow up. I just finished watching the video on melasma and was so amazed of the possibility that it could really clear up. It offered me hope! I’m now on my 17th week of NuDerm and although the melasma has lightened up, you can still see the blotches/discoloration. I’ve been on Tri-Luma previously and it cleared up a bit but returned. About 2 years ago a dermatologist advised laser treatment for the melasma but after spending all that money, it didn’t help. Nothing was ever prescribed to use on my face. I went to another dermatologist last year who put me on Tri-Luma, after that….it’s NuDerm.
I’m Asian, 59 yrs old and have suffered from melasma (although I didn’t know this until about 3 yrs ago) on my cheeks for about 10 yrs now. I’m so tired of using makeup to try to cover it up and in fact, now that it has lightened quite a bit, I’ve hardly used any makeup at all. offered Other than continuing on with the NuDerm and other products you think might address my issues (rosacea-like symptoms even at 16 weeks, dryness), what else should I do? Should I switch or add other products? I feel that I should continue on beyond 16 weeks and not go on a maintenance program yet. But could the continuous redness of my cheeks adding to the blotchiness now?
I also want to change dermatologist to someone who have had a lot of experience with melasma patients and the use of Obagi products. Can you suggest someone in the North County of San Diego area or close to it?
I know I’ve got a lot of questions for you but I’ve been seeking help for so long, going from one dermatologist to another, trying different treatment products or procedure. I’m becoming desperate now. Please help!

I would not recommend continuing with NuDerm past the 16 weeks thinking that your result will substantially change.

I’ve met and worked with Steven Cohen, MD, a plastic surgeon in La Jolla. I know he performs the Obagi Blue Peel, so you may want to check with him. The procedure is performed under a general anesthetic.

Jean–my educated guess is that the redness and blotchiness is not due to the products but may be an indication of rosacea. The cheeks and nose area are usually the first to show signs of redness and blotchiness. Have you had rosacea (which shows up as pinkish or reddish patches, and small red bumps) before? The condition can be successfully treated but not completely cured. Drinking alcohol, too much exercise, stress or UV exposure can trigger flare ups.
If there are no obvious factors and if the condition continues, discontinue use of the products. If the redness doesn’t clear up, see a physician. I’m reluctant to recommend products for the treatment of rosacea, because I don’t want to prescribe a cure for a problem that doesn’t exist. A dermatologist will be able to give you a more informed opinion.

Donna has sent me an email and I’ve already contacted your office for further info on Obagi Blue Peel. Due to some commitments, it seems like I would need to wait until June before I can have a peel. Meanwhile, is it possible to use Ommerse Daily Renewal Creme or the Overnight Recovery Complex in the morning instead of NuDerm Clear? I know that I must not completely stop the NuDerm regimen (while awaiting a peel) and thought that either one of these ZO products could possibly help with the issues of redness and dryness which have been a constant problem (in addition to my melasma, of course). I will continue the Clear, Blender/Tretinoin several times a week at night also as I don’t want to lose the progress my skin has achieved at this point. Jean–I highly doubt that the continued redness/peeling is caused by the products, but I’m happy that you’re seeing a physician. Before adding back some of the products, all of the products, none of the products, switching to some NuDerm products, etc., let’s get a professional opinion. We expect continued redness and peeling with NuDerm; it shouldn’t happen with ZO Skin Health products.

I have seen the dermatologist and he has asked me to discontinue all the ZO products for now. He had a look at the products ingredients and feels that some of the combinations could have trigerred the reaction causing my skin at the face and nect to be red, patchy, peeling and burning – he says it may create pigmentation problems. He has put me on a gentle cleanser, a steroid cream and a moisturiser for now.

My redness and patchy problems are gone and during my 2nd visit to the dermatologist after a week, he had asked me to stop the steroid cream but continue with the gentle cleanser and moisturizer as there is still some peeling. He also said that the skin may take another 2 weeks to settle down so there wont be any marks (caused by the sensitivity due to earlier problems).

The dermatologist said i may have been sensitive to Retinol and some other ingredients in the ZO line. I have not experienced these before with other skin/body/hair care productss. He only treated my irritation, no other recommendation for skin rejuvenation or restoration. I tried to submit a photo thru the MouseCall as you suggested but it hanged and wouldnt go through.
Can you advise if it is ok to take anti-histamines and use hydrocortisone cream for long term? He had given me another dose during follow-up visit this week as the irritation is still there.

Is the rash all over your face, or only in certain spots? It the irritation is related to the products, the rash will be widespread (covering all over your face, and whereever else it has been applied–like your neck).
In the other cases that I have reviewed, it looks like both women had some dermatitis, which could have been pre-existing and exacerbated by the product use.
You probably do not have a reaction to retinol. Do you have any history of eczema or psoriasis? Environmental allergies? Other product use aside from ZO? Your skin should have stabilized regardless of the oral medication you were given.
I would recommend you avoid any abrasive products (like the Exfoliating Cleanser or Exfoliating Polish) for now, until your skin is strengthened. Use a retinol product only every other day for a month. Don’t use products other than ZO–there could be an interaction. Use the hydrating cleanser (it’s very mild), Olluminate (if you wish), Daily Power Defense and sunscreen daily. If you’re using Growth Factor Serum, use it every other day, in the evening only. It should be applied to skin that has dried for a few minutes after washing. If you wish, use Ommerse on alternate days for a month.
Please be assured that there is no problem with products, I am sure your skin is just weak. Please stay in touch.

No, i do not have any prior conditions mentioned in your mail but if i recall correctly, many years ago i developed redness and swell after using a new skincare and was advised to stop (i returned it to the saloon). My rashes are across the face and neck, the 2 places i apply the ZO products but it is at localised areas like forehead, jaw, temples, chin and mid-neck. I am currently only on the local doctor’s cleanser and cream, i have stopped the anti-histamine and hydrocortisone cream as i believe it will not be good for long term.
From your recommendation above, are you suggesting i stop all the local doctor’s products and use the ZO products only as per the regimen provided? Am i understanding it right? If you think this will benefit me, i am willing to give it a try.

Hi, Jean: I thank you for the compliment, and while I can give your expert opinions about my own products–and many others–, it’s difficult–even impossible to be the expert on your skin. I have no idea what may be causing the continuing problems–all I can say is that it’s highly unlikely that it is retinol. Since we don’t know what is causing the reaction, we have no assurance that the same ingredient is not also contained in the products that your dermatologist has recommended. If you have confidence in him/her, I would suggest that you follow his/her directions–or, at least temporarily revert to products that you have safely and successfully used previously. Right now, we just want your skin to calm down.
It’s well known that my Obagi NuDerm products work by causing a reaction to the skin–dryness, irritation, peeling. In fact, they can’t work–at least initially–without it. But non-prescription products should not cause so much inflammation and discomfort.

I have been using the nuderm line since 6/ 08 and pleased with results. I am on maintenance now but I stopped using tretinoin bec of the burning sensation , redness and flaking. I found out of your ZO skin health line and I am interested in switching. I am 37, Asian. The dark spots in my face has lightend up but not yet completely gone , pores smaller but has black/whiteheads mostly in the nose area. Should I switch or use both? If I use both what regimen should I follow?

My name may be written on the bottle, but I don’t want to presume to know better than your physician, who has worked with you. Therefore, I can’t/won’t respond to your specific condition, but can speak generally.
Tretinoin/Retin A is still in my opinion one of the most effective and important products for anti-aging and skin health. Although frequency of application can be reduced to alternate days, seldom do I recommend that it be eliminated entirely.
ZO Skin Health products will not be as effective as the NuDerm regimen for treating pigmentation problems. They do not contain bleaching agents, or retinoic acid.
That being said, I do recommend that ZO Skin Health products be introduced into the maintenance regimen. Since the NuDerm regimen can’t and/or shouldn’t be used indefinitely, the ZO Skin Health products are an important part of the transition plan for a lifetime of healthy skin.
If you would like, someone on my staff can give you some examples of my recommended maintenance plan (through MouseCall (also on the website). Let them know what products you are currently using, and what your doctor has recommended. Although the maintenance plans are somewhat generic, they can be customized. Products these days are too expensive. Use only what you need, and what’s working.
Even with my own patients in Beverly Hills (and we take skin care here very seriously!) I’ve reduced the number of products that are recommended and/or their frequency of application.

As for the connection between hydroquinone and Obagi NuDerm… As you know, hydroquinone is a very effective bleaching agent for pigmentation issues–everything from freckles to much more serious conditions, such as melasma, age spots, etc. Currently, products that contain up to 2 percent hydroquinone may be sold in the U.S. without a prescription, and prescription products may contain up to 4 percent hydroquinone. It’s used in about 200 different products in the US–including NuDerm, TriLuma, etc.
As for you, I am very happy that you have had great results with my products, but I defer to your oncologist or dermatologist for further care. As the originator of “skin health”–that will be first concern. Take care of that, and we’ll take care of the pores and rosacea later.

I have recently purchased the Obagi Nu-Derm through my physician and today is the sixth day. I have not experienced any redness or peeling yet. I have had several chemical peels and Micodermabrasion done in the past. Could this make a difference? I’m using Retin A micro 0.1% with the blender. Should I be concerned that I’m not experiencing what I was told to expect? Or do I just need to give it more time? Donna has communicated with you about this, but just for the record, I can give my advice only generically–your physician will be able to give you advice as well.
I prefer Retin-A to Retin-A micro. The base is different, and while it may not be as cosmetically pleasing, overall produces a better result from NuDerm.
Oily skin can sometimes inhibit absorption of the products. If this is a problem, Accutane can be prescribed by your physician.
Finally, make sure that the NuDerm products are applied sequentially. Massage them gently until absorbed. You want the actives to be delivered to your skin, not air-dried on top of your skin.
I hope that one–or all of these– will make a difference for you. If you have other questions, ask your doctor. I would like to see you personally, and can try to help by cyberspace, but can’t beat the proximity of your own physician.

Seven years ago I used your NuDerm Regimen for the first time to treat age spots/freckles/hyper-pigmentation for normal/dry skin with 0.025%Tretinoin added. I tolerated all the products well, reducing most of the surface sun damage. I am English with red hair,fair skin with yellow undertones,freckles and rosy cheeks. At the time, I was 50 and although I always had normal/dry skin as I aged my skin became even more dry.
I am certainly thank you because it revealed some actinic keratosis spots and now I get check ups every 6 months plus I know what to look for so if a suspicious spot shows up I have it addressed right away.
After this NuDerm regimen program in 2002, my skin has continually to become more thin, lose its elasticity and youthful glow no matter what combination of products I tried. People always thought I looked younger than I am and suddenly no one did, especially me.
I was recommended using Vivete by Allergan with TriLuma for 3 months with Healthy Skin as my sunscreen. No adverse reaction except for the continual complaint that my skin was too dry, especially in the peri-oral region. After the 3 months I returned to your NuDerm with 0.05% Tretinoin. No adverse reaction other than the dryness which was not relieved by the addition of Obagi NuDerm 10% Vitamin C and Action. I tried alternating, laying off completely and then slowly easing back into the regimen which I was able to do but I continually had to use the NuDerm Action. Okay but not the optimal moisturization either you nor I expect to see. I was on this second round of NuDerm for 20 weeks.
I had a couple of Actinic Keratosis spots that needed freezing and I also decided it was time for a change in dermatologists. Considering the use of the Clear and the Tretinoin, I stopped using them 3 weeks before I was to be treated with liquid nitrogen. Fine with me because I used what moisturizing products I still owned as I knew how much my skin needed hydration and lots of it.
After all of the above, here is where I currently stand with the new dermatologist and what has been recommended:
(1) Come in for a Hydration Facial performed (granted, I of all people know that my skin desperately needs moisture and I love a facial but I have NO idea what this entails and if I am not following it up with daily moisturizing the facial will be short lived)but that’s just my opinion.
(2) Begin a maintenance program for 3 months using the following:
AM:
Obagi NuDerm Gentle Cleanser
Toner
Clear (alternate days)
Exfoderm (alternate days)
Healthy Skin Sunscreen
PM:
Obagi NuDerm Gentle Cleanser
Toner
0.05% Tretinoin (every night)

I agree with the scaled back approach above for a given time as I could tell, in just the short time I was off of everything besides the cleanser/toner that my skin needed hydration. I know my own skin very, very well and it needs daily moisturization without denying that the occasional facial is not out of line (and it’s so nice to be pampered!)
I would really like to incorporate appropriate moisturizing and nourishing product(s), that would not conflict or double up on what I am currently using.
I do want to go on record as thanking you from the bottom of my heart for your Obagi NuDerm exposing the existence of the Actinic Keratosis. It was always there because of the sun damage and your skincare regimen made me take quick action. It also taught me how important sun protection is and many, many thanks.
I well appreciate your position on not second guessing another physician and I am totally comfortable with that. I look forward to hearing from you and I hope that I can incorporate some of ZO Skin Health into my daily regimen. Someone else told me just this morning that our products exposed actinic keratosis, and prompted the woman (60 years old) to get prompt treatment.
You know the routine–the redness and irritation are an important part of the NuDerm regimen. That being said, here’s what we can do to make it more tolerable.
(and I appreciate the fact that you understand why I don’t like to second-guess your physician, so please just accept these as my own thoughts)
1. It’s good that you’re not using TriLuma in conjunction with Obagi NuDerm. It contains hydroquinone, Retin-A and hydroquinone, so would be too much for your skin.
2. About 90% of the improvement comes from the combination of Obagi NuDerm Blender/Retin-A and Clear; only 10% is derived from Exfoderm. If peeling and redness are really annoying you, you may be able to eliminate the Exfoderm. People who are very concerned about pore size and acne are more likely to require Exfoderm.
3. I recommend ZO Skin Health Ommerse Daily Renewal Creme in the morning to reduce the dryness. It will add to comfort, and not affect the result. Have you tried the ZO Skin Health Exfoliating Polish? It does a great job of clearing off the peeling (albeit for only a few hours). Use it in the morning on the shower–it’s easier to rinse off that way. For many people, the Exfoliating Polish enables them to successfully endure the Obagi NuDerm regimen.
In the evening, on the days you’re not using the Tret/Blender, Ossential Growth Factor Serum can be a good addition.
4. As for facials. Try to go six weeks without one. I generally don’t recommend them before this time. Then, if you choose to have one, cease the use of Tret/Blender for 2 days before the facial.
I hope that the addition of a moisturizing product (Ommerse) and the exfoliating polish will address your concerns. Don’t be concerned about it impeding the result.

Thank you for your recommendations which I am going to follow. Your explanation of cell turnover and skin renewal/rejuvination convinced me not to put the NuDerm Exfoderm away entirely as I do not want skin cell renewal to come to a screeching halt so I think using it 2-3 times a week maybe adequate for my skin type. I do want to keep the keep cell turnover going and I agree with the exfoliation of the dead cells enabling the absorption of your other skin care products.
Regarding my NuDerm Maintenance Regimen:
(1) I find my skin feels tight and dry after cleansing with the NuDerm Gentle Cleanser and following with the Toner. My skin should not feel like this after cleansing, correct? Should I switch to the Offects Hydrating Cleanser while in the maintenace phase?
(2) Still will continue to use NuDerm Clear and Exfoderm, as we previously discussed. Look forward to receiving the Ommerse Daily Renewal Creme and Ossential growth Factor Serum that I ordered and will use as you recommended. I also have Obagi 10% Vitamin C what recommended by my previous dermatologist to help counteract my dryness. What is your recommendation for that serum as of right now?
(3) My eyes..Two issues here are: dry,crepey eyelids which droop now thanks to Mother Nature and undereye bags which I have had for a long time and they have gotten worse too naturally. Currently I use two products:AM- Obagi Elastiderm creme (okay but not estatic) and PM- MD Forte Skin Rejuvenation for Eyes (good but I would rather be using your line). As for the bags, I am realistic that nothing topical is going to reduce them to perfectly youthful appearing eyes. I have no probelm with tolerating the retinoid products so what would advise here?

I’m glad that you’re happy, and doing well with the products. In answer to your questions:
(1) It’s OK to switch to the ZO Skin Health Hydrating Cleanser. No problem there. Your skin will begin to feel tighter, but that’s partially because it is tighter–you have enhanced the production of collagen and elastin, so your skin is getting firmer.
(2) Initially the ZO Growth Factor Serum and ZO Daily Renewal Creme may cause some stinging/tingling–depending on how much of the redness and irritation has abated. The Vitamin C should be applied before the Clear, in the morning. I would recommend that you stick with the 10% (since that’s what your doctor recommended), but let’s consider the 15% or 20% after you complete the regimen.
(3) Aaahhh, I wish I had some good news for your eyes. But before we talk about the treatment, have you thought about the cause. Could it be that you’re over-tired? (you know the remedy for that) Could it be an allergy? (are you taking anti-histamines?) Do you rub your eyes (they’re very sensitive). Do you have too much sodium in your diet? Are you dehydrated? Sometimes your eyes will retain and accumulate water as a defense mechanism. Is there possibly a simple cure??? Have you considered refrigerating your eye cream. Sometimes the cool product is enough to reduce the swelling of tissues (albeit only temporarily). There’s something called “Hollywood’s Secret”–are you familiar with it? Rumor has it that Preparation H (which shrinks swollen tissues) is very effective for puffy eyes, but the manufacturer (Wyeth) does not recommend it. (So I won’t either!) The current state of the art is to inject Juvederm or Sculptra–injectible gels. Of course, I can’t say that I would recommend that for you, without first exploring other options.

So the tightness I am experiencing after using the NuDerm Gentle Cleanser, in particular peri-orally is normal? I like the idea of my skin becoming firmer thanks to the NuDerm regimen.
I was concerned that the tight, dry feeling was due to the Gentle Cleanser and when I proceeded with the next step, the Toner my skin did not feel any better. At this point, it feels in great need of some moisturization because it feels really tight.
That is why I was thinking perhaps I needed a more hydrating cleanser but the bottle of NuDerm Cleanser and NuDerm Toner are fairly new so if what I am experiencing is normal I stay the course and see it through.
After cleansing and toner in the AM while on NuDerm Maintenance:
ZO Exfoliating Polish (twice a week) otherwise, Clarisonic (if peeling?)

Back to the eyes, my jar of Elastiderm Eye Cream is almost empty. Which do you recommend? Elastiderm Eye Cream or the ZO Ommense Eye Creme?
Please explain the differences between the two for me so that I can make an informed decisison. I see in the Elastiderm pamphlet it states that it is not a moisturuzer which left me somewhat confused…
And I will start leaving my eye cream in the refrigerator. That ought to really throw my husband for a loop so as long as it does not look like pesto sauce or roasted minced garlic it will stay put. Although he is bound to ask me what it is which should prove interesting.
As to the reasons for the undereye puffiness: It’s partially hereditary but I do have allergies so I use oral and nasal spray antihistamines. The amount of sleep I get is erratic and I do not know why. I drink 64-96 ounces of water each day, do not like salt or soft drinks or fast food.
The skin around my eyes is fairly smooth with minimal crow’s feet. The only wrinkles exist at the bottom of the underyeye bags and a bit where the upper eyelid is somewhat droppy. Question: Do I apply the Eye Cream on the eyelid itself in order to help improve the creppiness or just lightly tap the eye cream on the orbital bone in a “C” shape?
Inform me please as to the differences between Elastiderm and the Ommense Eye Cremes as I will be in need of one soon.

I’m now ready and seriously considering the Obagi Blue Peel for my melasma but have concerns because I have sensitive skin such as almost looking like I have rosacea (very red) everytime I wash my face (I use NuDerm gentle cleanser and wash my face very gently) and when the weather is hot and humid. Would it be better to have what they call a Sensi Peel (supposed to be designed for sensitive skin or those with rosacea) first to see how my face reacts before going for the Obagi Blue Peel? Appreciate your advise.

In this forum, I’m reticent to give you advice, because that’s the role of your physician. That’s why it’s written somewhere on here that it’s intended for non-medical questions and concerns.
All of that being said, I can tell you that I have successfully treated many, many diferent skin types with the Blue Peel.
I don’t know what is causing the condition that you describe as sensitivity. You may want to meet with one or more physicians to find out what their explanation is for the probable cause of the sensitivity, how much experience they have treating melasma with the Blue Peel. You can find a list of Obagi physicians at obagi.com, although call the doctor’s office first to see if they also perform the Blue Peel–not all do. It’s an outpatient procedure generally performed under a local anesthetic.
Myrna, while I would like to be of more assistance to you, I don’t want to do a disservice by improperly or incompletely diagnosing your skin. Keep me informed, though, and let us know what you decide and how you do!!!

Thanks for your nice words. I love treating skin and I love developing products, so it is both a vocation and avocation of mine.
Without Exfoderm, the eventual result will be the same, but it may take a little longer. The combination of Retin-A and Blender is the real workhorse. Think of Exfoderm as a jet vs. a plane. It makes the journey go a little faster, but the destination (the eventual result) is the same. For people who can tolerate it, it’s great. But it’s not absolutely necessary.
The Clarisonic (and the Exfoliating Polish) may improve comfort and appearance and give a temporary respite from the peeling and flaking. In the very beginning you may find both difficult to tolerate. Use it when you’re ready. It will neither accelerate nor retard the result–these exfoliants work from the outside–think of them strictly as a polish– while NuDerm works from the inside. The thing I like about the Exfoliating Polish is that it has round magnesium crystals that are less likely to wound your skin, which is understandably sensitive at this time.
So there’s a cosmetic benefit to the Exfoliating Polish, but there’s also a clinical benefit. If you can clear the skin of the loose, flaking, dead skin cells, you will enhance the penetration of other active ingredients. I like to think of skin as a piece of canvas, and you are the artist. If you can clear the canvas (your skin) of dust, particles, etc., you will enhance the absorption of the paint (the creams). But in the first couple of days–maybe for the first 2-3 weeks–while irritation is at its peak, treat your skin gently.
I have patients who tell me that they couldn’t tolerate the NuDerm regimen without the occasional use of Exfoliating Polish. It begins to show them how good they will look at the conclusion of the treatment. I have others who tell me that they just want to try to ignore the peeling and flaking, and avoid looking in the mirror for 8 weeks. Do whatever works for you.

Moisturizers are only harmful with they are used to the exclusion of products that activate and stimulate the skin. Think of them like “dessert”–moisturizers deliver pleasure and instant gratification, but don’t give your skin the “kick” that it needs. So the skin cells get lazy, and go dormant.
If you are less than 30, your skin cells are probably naturally quite active. Over age 30, they begin to go dormant, and slow the production of collagen and elastin. That’s when you really need to wake them up, and shake them up.
So in answer to your question. No, don’t throw your moisturizers out the window. Just be sure that you are using a moisturizer in conjunction with products with retinol, peptides, Vitamin C, anti-oxidants—the products that really work for your skin.

I’ve been using Nu Derm since 2002, I’m very happy with my skin, I’m 46 years old and people always think that I’m in my 30’s. I don’t have wrinkles nor freckles, my pores are small, my skin is very smooth. I regularly do cooltouch3 every 3 months for the past 4 years now. I also do light peel every 3 months at the obagi clinic here in Jakarta. I’ve been told by people that you should not use Obagi Nu Derm every night for too long period of time, but my dermatologist at the Obagi clinic told me it’s ok to use it every night as long as the ratio between Blender and Skin Enhancer(tritenoin) is 1:1/2.
My question is, is it safe to use the Nu Derm blender + skin enhancer every night for the past 7 years now? I wanted to change it to Zo Radical Night Serum and Growth Factor Serum which I bought a month ago, but everytime I stop using NuDerm for more than 3 days my skin starts to have pimples again just like before I started using Obagi in 2002. What should I do doctor, can you help me? Can you also recommend an Obagi dermatologist in Singapore that I can consult with for second opinion?
At the moment I am very happy with the result of your product, everywhere I go people compliment me with my skin and I always tell them that I use Obagi.

I saw the picture you supplied, and your skin appears to be in excellent condition.
Your physician is the authority on your own skin care, so I will speak generally. I don’t want to try to second-guess your doctor.
Long term daily use of Obagi NuDerm Clear and NuDerm Blender is not required. It’s not unusual with Rx’s– certain drugs (like antibiotics and steroids) cannot/should not be used on a continued basis. “More” does not equate to “better”.
Once someone completes their initial treatment program (approximately 18 weeks–maybe more, maybe less), NuDerm has kickstarted the skin. At that time it’s important to go into the “maintenance mode”, and introduce ZO Growth Factor Serum or Radical Night Repair. It’s important to continue to stimulate the skin (to keep that collagen and elastin pumped up).
If you try cutting NuDerm out completely, you can expect a return of pigmentation issues/discoloration/age spots.
As for an Obagi physician in Singapore, yes, I will have someone get a recommendation for you.

I am Eurasian, 38 years old, living in Bangkok. I have melasma on my cheeks. I’d like to start using the Obagi Nuderm line, I believe the entire course is 18 weeks. Can you advise how I should use that afterwards for “maintenance”. I’d like to incorporate your ZO line while tapering off the use of NuDerm. Kindly suggest an AM/PM regimen that would suit my need. My skin type is oily on the T zone. Large pores, I’m seeing a little bit of skin sagging on the jowels and dark under eye circles. But so far, I haven’t seen any wrinkles on my face just yet. I have to apply foundation to my face everyday to cover the melasma on my cheeks and pretty much uneven skin tone all around.
By the way, if I start using Obagi Nuderm, can I do laser such as YAG or IPL, will that cause too much irritation or dryness?

I have used your Nuderm products for approx. 12 years now and have been very happy. I started using it b/c of acne problems ( I have very oily skin).
About four years ago I developed melasma. On and off, I went through the Nuderm aggressive protocol, which included .1% retin- a and have had a few fraxel treatments over the past year and a half. Nothing seems to be happening with the melasma. I am wondering if I am being too aggressive with my skin, thus worsening the melasma. Currently, I am using the following products after being on the maintenance program for a few months ( I mostly stay with maintenenance):
AM:
IS clinical super serum
Obaji clear and exfoderm
PM:
Neocutis bio gel (Similar to TNS Recovery complex)

There are several different types of melasma–some more difficult to treat than others. Both kinds are exacerbated by UV rays, pregnancy, and birth control pills, and affect the center of the face–the upper lip, nose, chin, forehead and cheeks. Epidermal melasma (which is more superficial) is commonly treated with Obagi products (a combination of tretinoin and hydroquinone), a good sunscreen, and patience–it can take 18 weeks or more. But the response and results are generally excellent. Sometimes a Blue Peel is required, but not always. Dermal melasma, on the other hand, resists conventional treatment and may require multiple peels and laser treatments. It’s easy for a physician to determine the nature of the melasma–just by using a Wood’s Light. Have you had an exam?
As for your AM regimen–I hope its just an oversight that you didn’t mention that you’re using a good sunscreen. It’s critical. As for your PM regimen, it’s fine.
You can stay on the maintenance regimen for 3-6 months (on that you can use the Obagi NuDerm products just 3-4 days a week, then substitute ZO on alternate days). Then revert to the active (what you describe as aggressive) treatment for 2-4 weeks. STay with it for only two weeks if your skin is in good condition; continue with it for a total of 4 weeks if your skin was damaged on vacation, etc.

I have been using the Nu- Derm System for 10 weeks now. I am using the whole system on my entire face but I am just using the blender and tretinoin on my chin area to treat acne scars and acne. My chin area is red and a different color than the rest of my face. I can see a line around my chin where I am using the blender. I just want to make sure that the color will even out with the rest of my face! My skin tone overall is amazing but I still have a couple brown spots around my chin area that are fading slowly but surly I’m just very nervous that the discoloration around my chin will not go away. I used all of the ZO products and whilst they were great (Growth serum particularly) I wanted something a little more aggresive on the few wrinkles I have. I did not find the eye cream nourishing enough and it left my eyes a bit crepy.
I have a friend who uses and raves about Obagi Nu derm so i swapped 2 weeks ago waiting for the peeling but NO!! I am using gentle cleanser, toner and then followed by Exfoderm. For 2 weeks now I have had a few tiny whiteheads appear everyday (I never get spots!) I have carried on with the treatment but now feel as though I am wearing someone elses skin on top of mine and have no peeling. I do have dry/sensitive skin normally.
I also bought the Elasiderm eye treatment for day and night but am allergic to it. It brings my eye lids out in a stinging rash although the under eye seems to be ok.
Can you help and advise as I really would like skin like by Obagi friend!!

I’m glad that you liked the Growth Factor Serum.
As I oft-repeat in this blog, it’s difficult for me to give you product recommendations via cyberspace, but I can ask some questions.
There are many products in the NuDerm line–some much more aggressive than others. So I need to know more–what products are you using? The cleanser and toner won’t cause peeling and certainly wont cause whiteheads. The heroes of the line are really the combination of a tretinoin and Blender in the night–that’s what causes the peeling. What are you using in the evening? Are you under the care of a physician?
As for the Elastiderm–you may or may not be allergic to it. I wouldn’t recommend using it in combination with the NuDerm because during the treatment phase, your skin will be much more sensitive than usual. The skin around your eyes is already (under normal conditions) very thin and very sensitive, and will be easily irritated (stinging) by the NuDerm. When you have completed the treatment, you may want to use a hydrating serum (this is similar to a hydrating masque, but is designed for the periorbital area). You can use it just a couple of times a week.

I am 32, and have normal/combination skin. My skin is starting to look duller and less bright, darker too even though i don’t have more sun exposure. One thing that stands out immediately is that you are using far too much moisturizer to the exclusion of products that activate and stimulate your skin. Moisturizer inhibits the natural exfoliation process and that causes the skin to appear dull, as dead skin cells accumulate on the surface. Further, with some retrospective analysis we have done in our clinic, we’ve found that those patients that are “frequent” users of moisturizers tend to either have acquired sensitivity (not from a true medical condition such as contact dermatitis) OR tend to accelerate the aging process in skin. Moisturizers alter the natural lipid barrier in skin, fooling surface skin into feeling superficially hydrated, thus sending a signal to source cells not to regenerate. Overtime, this causes one to feel like they “need” a moisturizer and for skin to become rely on one for hydration.
Pigmentation changes and scars require stimulation, and you must go through some exfoliation, peeling, and dryness phase. This exfoliation and dryness is temporary as skin will build tolerance, where you no longer react but continue to achieve correction. Over the long-run, your skin cells will be active and you will feel naturally hydrated again. A less aggressive stimulation program is the ZO Skin Health system, while a more aggressive medical grade program is the Obagi Nu-Derm system. The following Nu-Derm products can be used around the eyes (under eyes and eyelids), the Clear, Blender & Tretinoin (very thin layer), Cleansers, All Eye Creams (ElastiDerm, Eye Cream).
The Toner, Exfoderm, and Pro-C Vitamin-C serums can be used under the eye, but should not be applied on the upper-lids.
The eye area has much thinner skin; therefore, 2-3 times a week of application of the Tretinoin & Blender is sufficient.
Dr. Obagi does not like his patients to use the Retin-A Micro with the Obagi Nu-Derm system. As a result of the delivery system and timed release, correction is limited to acne. It will not improve elasticity, tighten skin or stimulate collagen as well as Tretinoin Cream (generic for Retin-A cream) does. The preferred concentration is 0.1% cream (red and white tube). Stick to the program, despite the temporary reaction, you are going to get excellent results.

I live in Canada, Ontario Toronto, and i got the Obagi Nuderm through my dermatologist and I loveeeee the products because i saw my sister using it and her skin looks very pretty, raidiant, even skin tone and no breakouts at all.
i use the obagi products all over my face and this monday april 12, 2010 will be my 2nd week completed.
these are the steps that i was told to do:
Nuderm products:
Step 1: Foaming Get -wash whole face and then pat dry.
Step 2: Toner- pour onto cotton ball for the whole face except eyelids.
Step3: Clear- Put pea size amount on whole face + eyelids.
Step4: Obagi Professional – c Serum 10% and after use Exforderm – put pea size amount on whole face except eyelids. *exfoliation may cause burning sensation*
Step 5: put sunblock on 10 minuts prior to going outside.
NIGHT TIME:
Repeat steps 1 to 3 and skip step 4 (no C-Serum and no Exfoderm for night time) and do step 5: Blender + retin A put pea size amount on whole face and eyelids**exfoliation and redness occurs**
(STOP step 4+5 for 2 days only if burning sensation occurs) (Apply Moisturizer: ALYRIA only when burning / redness or peeling occurs in AM/PM)
My questions:
1) i was also told to use clear, blender and retin A 0.1% all over my eyes(under and above)with an eye cream. so i started doing that but i’m getting alot of wrinkles and i’m also getting a long wrinkle under my eyes which extends to my check. should i be using these products around my eyes? if not then should i only stick with an eye cream and will my wrinkles go away? I mean, i would love to use the products around my eyes if i will see improvements if not then which eye cream u would recommend to fix this?. My conern about my eyes are lines, wrinkles, and dark circles and i only use a little amount of these products around my eyes.

I have been on the NuDerm for a year now. I initially used all products last October and did the entire regime. Worked great. My skin care specialist said I can stay on the products, just use the Tret and blender 1 or 2 x’s a week now. I use the gel cleanser each day and night. My first question is, is this ok to say on for the rest of my life. I apparently have the “mask of pregnancy” or hyperpigmentation problem that is why I stated to begin with. I have noticed now a few red patches occurring on the top of my cheek bones, could this be from dry skin?
Also, I recently purchased the Elastiderm Day and Night Cream. My eyes looked great before using it. It has been a week and it looks as though my wrinkles are really really showing. I know from teh NuDerm system that it made me look worse befor better, is this the same with the eye cream? Also, should this product be for someone older than 38?
Please let me know your thoughts.

Melasma and most forms of brown spots are a chronic condition. Since you are prone to forming these as a result of hormonal changes (those that occurred during pregnancy), you should be on a “maintenance” program in order to manage your color cells (melanocytes) as they can reoccur in instances such as, other hormonal changes (birth control pills, natural changes), sun, or trauma.
The Tretinoin & Blender step is recommended 2-3 times a week for patients with a history of brown spots. The Clear should be used at least 3 times a week as well, as patients use the Clear 3-4 times a week if there is a history of brown spots. If you were using the Exfoderm, it too should be used 3 times a week. Pro-C (if used) can be utilized daily. Cleanser, Toner, Eye Cream, and Sunscreen are used daily.
In general, the Elastiderm Eye Cream (not the gel, for your skin type) does not cause the dryness or appearance of more wrinkles that you are referring to. It is a strong eye cream, and if your skin is not accustomed to it, you can experience some temporary irritation. What you can do is reduce its use to every other day or less, until you slowly build tolerance or become adjusted to it. Then increase as you are able to. Reducing your program to the “Maintenance” routine will not cause the red/dry patches you are referring to. Just be careful when cleansing your skin to gently do so using only your finger-tips. Do not attempt to force or roll any dead skin off, as this can cause red or raw patches. With the weather change, it is normal to become drier… To alleviate any dryness, irritation and provide you some great “functional” hydration creams consider incorporation the Ommerse Daily Renewal Crème in the AM prior to applying sunscreen and the Ommerse Over-Night Recovery Crème in the evening as your last step at night. These are used both on the days you use your Obagi Nu-Derm creams and the days you are off them during maintenance.
These 2 steps hydrate, reduce inflammation, and promote the growth of healthy skin cells. Please read up on them on our webpage and let me know if you have any other questions. FYI… 38 is not too young for the Elastiderm or Olluminate. We begin to lose collagen at a rate of 1% per year starting at age 28.. It will reverse this trend. Thanks.

Please enlighten me on the use of retin a. 0.1 strength was prescribed together with the Nu Derm set I bought. I was told that ALL of the clear, blender, exfoderm are not to be used under the eyes. However, its my undereye area that needs correction because there are lines already. Can I use the blender with retin a? or only the retin A to correct it?

Thank you for emailing. I have answered this same question on may occasions and completely relate to your confusion. If you look over all of my responses throughout this site, you will see that both Dr. Obagi and I advocate use around the eyes.. This is where skin is usually most damaged, with respect to fine lines and wrinkles.
Lets clarify the correct usage of each item.
Cleansers: Used all over the entire face, including eyelids.
Toner: Used UNDER the eyes and the entire face (no eyelids).
Clear: Used over the entire face INCLUDING under your eyes and your eye lids.
Exfoderm: Used under the eyes (avoid the eyelids)
Blender & Tretinoin Cream: Used around the entire eye area as well 2 days a week. The skin around the eye area is much thinner (epidermis) in comparison to the skin on the rest of the face (such as the cheeks). It takes a lot less product to induce correction, thus results.. Also, a lot less product induces the temporary reaction associated with the entire Nu-Derm system.
Eye Cream: An excellent idea to complement/enhance results and provide comfort. The best ones for anti-aging are either the ZO Skin Health Olluminate (especially if you also have dark circles) OR the Elastiderm (Obagi Nu-Derm). Use 2xs a day, before sunscreen in the AM and as your last step during your PM routine.
Remember, as with the face, the skin around the eyes may TEMPORARILY look worse, as the program dries skin out to shed damaged layers and stimulate the growth of new cells and healthy layers. The dryness causes the appearance of fine lines and wrinkles to be more prominent. You are NOT getting more wrinkles. If you do become overly irritated around the eyes, back off for a few days then resume again.
You don’t need to saturate the eye area with product. Apply to your entire face first, and spread in a thin layer around eyes.

I have been using Nu-Derm for 6 weeks and I started after 2 days peeling and redness. Everyday around 5PM i get a hot red face and it is so dry and uncomfortable. Am I having an unusual reaction? The only relief I have found is taking an aspirin and holding a cold compress on my face. If I go anywhere I am asked why my face is so red. I feel like I need moisture but all I read from your blogs is that is not what I am supposed to do. It is also hard to get the sunscreen to blend in because of all the peeling so I am having to pat it on. My sunspots are going away but my cheeks, nose and chin look awful. What can I do?

Can you please tell me the concentration, amount, and frequency of application of your Tretinoin Cream? What you are describing is normal, so don’t worry. The flushing, warm sensation, irritation, etc., is a side effect of the Retin-A® (Tretinoin Cream) – which in your specific case, is slightly above average. We can easily manage this.
Let’s establish some basics. You should be using the cream form of Tretinoin in the 0.1% or the 0.05% concentration. No Renova®, Triluma®, Micro-Release, gel, etc.. Alone these are great products. However, mixing with Blender (or Obagi Nu-Derm in general), is not recommended, as they will cause more irritation than the cream form of Tretinoin.
Most physicians have patients start by using either 1/2 gram or 1 gram every single night mixed with 1/2 gram of Blender. If extremely sensitive, reduce to 2-3 times a week only at 1/2 gram + 1/2 gram mixture. Yes, we state NOT to apply a generic or conventional moisturizer that inhibits the growth of new and healthy skin cells (and slows down the removal of damaged ones). However, the Obagi Nu-Derm Action cream or the Ommerse Day or Night creams by ZO Skin Health are perfectly fine to use, especially with the reaction you are describing.
For patients that experience what you are describing, I have them use the Ommerse Daily Renewal Crème every morning PRIOR to the sunscreen, and EVERY night as the last step. You can apply on TOP of the Tretinoin & Blender those 2-3 days a week and on the OFF days from this step, apply after the CLEAR. Ommerse enhances results, reduces irritation, and should not slow down your program too much.
If you are also using the Exfoderm, reduce this to 3 times a week as well. Again use the Ommerse on your off days in the morning. Start this specific protocol after 2-3 days of rest from the Tretinoin & Blender step. In other words, stop your Tretinoin & Blender step for 3 days, then resume with the slower protocol just outlined. Use less of this step around your mouth. Also, only apply around your eyes 2 times a week.
Don’t rub skin too hard when apply creams. Only use your fingertips to gently massage into skin until fully absorbed. Once you feel that you have built “more tolerance” to this protocol, we can increase your frequency of Exfoderm and the Tretinoin + Blender step. This usually occurs after 1 month of use. Thanks!

Hi dr Obagi, I’m Indonesian with medium skin tone and want to lighten and brighten my skin tone. The thing is I get dark easily. What product should I use? Pro-c serum or c-rx serum? I heard that c-rx serum is prohibited in my country since it has hyroquinon in it.

Hydroquinone is NOT prohibited in your country. You used to be able to purchase it without a prescription. Now, in the same way as the USA, you must have a prescription to purchase. To obtain a prescription, you have to see a doctor for it. There are several Obagi Centers in Indonesia that carry both the Obagi Nu-Derm system and the ZO Skin Health line. Depending on how severe your brown spots are or discoloration, you may benefit from a combination of either the Obagi C-Rx and ZO Skin Health together, OR the Obagi Nu-Derm and ZO Skin Health together. Just make sure to always wear a good sunscreen such as, the Oclipse and reapply every 3 hours if out-doors. I am going to have our Indonesian Skin Health Consultant, Cherry, reply in Indonesian as well.

I live in Canada, Ontario Toronto, and i got the Obagi Nuderm through my dermatologist and I loveeeee the products because i saw my sister using it and her skin looks very pretty, raidiant, even skin tone and no breakouts at all.
i use the obagi products all over my face and this monday april 12, 2010 will be my 2nd week completed.
these are the steps that i was told to do:
Nuderm products:
Step 1: Foaming Get -wash whole face and then pat dry.
Step 2: Toner- pour onto cotton ball for the whole face except eyelids.
Step3: Clear- Put pea size amount on whole face + eyelids.
Step4: Obagi Professional – c Serum 10% and after use Exforderm – put pea size amount on whole face except eyelids. *exfoliation may cause burning sensation*
Step 5: put sunblock on 10 minuts prior to going outside.
NIGHT TIME:
Repeat steps 1 to 3 and skip step 4 (no C-Serum and no Exfoderm for night time) and do step 5: Blender + retin A put pea size amount on whole face and eyelids**exfoliation and redness occurs**
(STOP step 4+5 for 2 days only if burning sensation occurs) (Apply Moisturizer: ALYRIA only when burning / redness or peeling occurs in AM/PM)
My questions:
1) i was also told to use clear, blender and retin A 0.1% all over my eyes(under and above)with an eye cream. so i started doing that but i’m getting alot of wrinkles and i’m also getting a long wrinkle under my eyes which extends to my check. should i be using these products around my eyes? if not then should i only stick with an eye cream and will my wrinkles go away? I mean, i would love to use the products around my eyes if i will see improvements if not then which eye cream u would recommend to fix this?. My conern about my eyes are lines, wrinkles, and dark circles and i only use a little amount of these products around my eyes.

You should not use Pro-C, Exfoderm, or Toner on your eye lids. Under your eyes is fine. Retin-A and Blender (make sure it is the cream form of Retin-A or Tretinoin in the 0.1% or 0.05%) should only be used around the eyes 2 times a week. The Pro-C is used after the Toner. It is applied before CLEAR, not after or before Exfoderm. 1) Cleanse 2) Toner 3) Vit-C, 4) Clear, 5) Exfoderm 6) Ommerse and Eye Cream 7) Sunscreen Your evening program looks fine, just incorporate the Eye Cream and Ommerse into your program. We want you comfortable while achieving results.
An Eye Cream such as, the ZO Skin Health Olluminate or Obagi ElastiDerm Cream (not gel) is an excellent idea to enhance results, stimulate collagen, and reduce irritation from the use of Retin-A and Blender. Using Retin-A cream around the eyes will temporarily make wrinkles around your eyes appear worse, as fine lines and wrinkles are enhanced when skin is dry. You probably over did it, by applying daily. Just stop around your eyes for a few days, and when you resume only apply 2 times a week. Eye Cream can be applied two times a day. In the AM apply before sunscreen and at night, apply as your last step.
Think of moisturizers as a dessert… They should not be relied on solely to deliver nutrients and stimulate skin (the same way dessert- shouldn’t be viewed as your daily meal). Occasional use is fine, especially to alleviate occasional dryness, to have a better base for makeup, or to enhance skin for a special occasion.
I am quite fond of the ZO Skin Health Ommerse Daily Renewal Crème and the Ommerse Overnight Recovery Crème, as these are “functional” moisturizers – providing stimulative benefits to the skin, while hydrating skin. These can be incorporated into your current program, without any reservations. Based on your age, you should apply the Ommerse Day Crème after your Retin-A and Blender step. If you are dry in the AM, apply before sunscreen (same time as Eye Cream). We use 1 gram of Clear (amount of toothpaste on a toothbrush), ½ gram of Exfoderm, 6-8 drops of Vit-C, and ½ gram of Blender and ½ gram of Retin-A. However, confirm with your doctor first. Congrats on your wedding.
